Single Individual
For accounts opened in the name of a Single Individual.

Multiple Individuals
For accounts opened in the name of Multiple Individuals.

Sole Proprietorship
For accounts opened in the name of a Sole Proprietorship not operated as a corporation, limited liability company, partnership, or trust.

Corporation
For accounts opened in the name of a Corporation.

Limited Liability Company
For accounts opened in the name of a Limited Liability Company.

Partnership
For accounts opened in the name of a Partnership.

Some supplemantary forms may need to be downloaded and filled out.
Please download and fill out if any of these apply:

• Cooperative--If the organization opening the account operates as a cooperative it may be eligible to become a member of FCStone Group, Inc., formerly known as Farmers Commodities Corporation.
• Commodity Trading Advisor("CTA")--If the account will be traded by someone other than the account owner, and the trader is registered as a Commodity Trading Advisor ("CTA")
• Discretionary Account--If the account will be traded by someone other than the account owner, and the trader is NOT registered as a Commodity Trading Advisor ("CTA").
• Non-US Customer--If the account is owned by a non - US customer.
• Transfer--If the account is currently with another brokerage company and the owner wishes to transfer the account to FCStone, LLC.
• Futures Direct Account--If the account is being opened as a Futures Direct Account.
• Specialized Brokerage Account--If the account is being opened as a FCStone's Specialized Brokerage Account.
• Customer Information Change Form--An existing customer or client should download this form if they need to change their contact information.
